<p><b>A: </b>There are laboratory tests in the lab that are not just plain testing. Like for HIV..nowadays the technology can not be fooled. At the lab, HIV testing is so important (it is like risking our license if we release inaccurate results ), Aside from the usual test (where we use high-tech machines )we do also confirmatory tests. So basically, factors that are affecting the accuracy of result may be eliminated as the blood undergo the procedures. Well, as mentioned there may be small percentage of errors..but rare to occur. <p><b>Q: </b>Do Antiretroviral drugs make HIV less contagious?<br />I am finding nothing about this in popular HIV literature. Scientific articles suggest drugs DO make it less contagious. I asked a well educated gay guy and he didn&#8217;t know. If these drugs DO make it less contagious, is this being kept a SECRET to keep people scared and more cautious? That may be a mistake as an article in The Economist (see free archive, articles by subject) says a big reason HIV is still not declining greatly is that people are afraid to get tested&#8230;Interviewees say they fear discrimination if they test positive but I suspect they also want to keep enjoying unprotected sex.  Maybe if they knew therapy would make them less contagious they would fear testing less?&#8230;Wierd logic but human.</p>
<p><b>A: </b>You are right, though contagious is not the right word&#8230;.infectious is more appropriate.</p>
<p>Antiretrovirals interupt and/or prevent HIV from replicating in the body. This reduces the amount of virus in the blood (viral load). The goal of therapy is to reduce the viral load to such a low level that it is undetectable (still there, but so few viruses they are not detectable when tested.)</p>
<p>We don&#8217;t know everything about the relationship between viral load and transmission, but we do know:</p>
<p>1. In general, the higher one&#8217;s viral load the more likely one is to transmit the virus and the lower one&#8217;s viral load the less likely one is to transmit the virus.  </p>
<p>If a person is NOT on HIV meds and their viral load is below 1500 it is rare for infections to occur. If a person&#8217;s viral load is low BECAUSE they are on meds, see #2</p>
<p>2. A person with an &#8220;undetectable&#8221; viral load in the blood is still infected with HIV, can still have higher levels of virus in their genital fluids (especially in the presence of sexually transmitted diseases which can result in sharp increases in HIV viral load) and can still infect others.  Cases have been recorded of transmissions with an undetectable viral load.</p>
<p>3. Effective Highly Active Anti-retroviral Treatment (HAART), taken properly, usually significantly decreases the amount of viral load. Despite these lower viral loads, increases in unsafe sexual behavior have actually caused the rate of HIV transmission to rise in recent years. </p>

<p> CCR5 antagonists:<br />
In order to enter a human cell, HIV must first attach itself to proteins on the cell’s surface. The virus always begins by latching on to a protein called CD4. The next stage involves proteins called co-receptors, of which there are two main types: CCR5 and CXCR4. Some strains of HIV use CCR5, others use CXCR4, and some can use either.</p>
<p>CCR5 antagonists are drugs that bind to the CCR5 co-receptor so that HIV cannot exploit it to gain entry to a cell. The main drawback of these drugs is that they don’t work against all strains of HIV.</p>
<p>Most people newly infected with HIV carry strains that only use the CCR5 co-receptor. As time passes the virus tends to diversify, so that around half of people in the more advanced stages of HIV infection have strains that can use CXCR4. So-called tropism tests can distinguish between the two types of virus, but these sometimes fail to detect low levels of the CXCR4-using strains.</p>
<p>Maraviroc became the first CCR5 antagonist to gain FDA approval in August 2007, and was approved in Europe the following month. This drug – marketed as Selzentry in the US and Celsentri in Europe – comes as tablets to be taken twice per day. Maraviroc is only approved for use in patients who have exhausted other treatment options, and so far sales have been lower than expected, largely because of the need to use tropism tests with relatively high rates of error. A more accurate tropism test was introduced in June 2008, and if maraviroc performs well in longer-term use then it may become more popular. There is even a chance that the drug could be approved for first-line treatment.</p>
<p>Vicriviroc is a similar drug undergoing Phase III trials in both treatment-experienced patients (that is, those who have already used other antiretroviral medications) and patients new to HIV therapy. The first studies were due to end in mid-2009. An earlier trial of vicriviroc raised concern that it might increase the risk of cancer, but larger studies have helped to allay such anxiety. It is likely that vicriviroc tablets will be suitable for once-daily dosing.</p>
<p>PRO 140 is in Phase II trials and is therefore a long way from approval. PRO 140 contains genetically engineered antibodies, similar to the proteins the human immune system employs to fight infections. This means that PRO 140 must be injected, or else it would be destroyed in the stomach. Because it remains in the body for a long time, PRO 140 may have to be injected only once or twice per month. Compared to maraviroc and vicriviroc, PRO 140 seems to have less impact on the useful functions of the CCR5 protein, which may mean it has fewer side effects.</p>
<p>Anti-CD4 antibodies<br />
TNX-355 – also known as ibalizumab – blocks HIV from entering cells by binding to the protein CD4 on the cell surface. Like PRO 140, TNX-355 contains antibodies and is injected once every two weeks (or possibly even less often). A concern is that interfering with the CD4 protein on immune cells may impair the body’s ability to fight disease, but so far no such effect has been seen in studies. As with other injected antiretrovirals, the market for TNX-355 is likely to be small; as of August 2008 it was uncertain whether the manufacturer would continue with Phase II trials.</p>
<p><b>Q: </b>Is it possible for an HIV positive mother on antiretroviral drugs to breastfeed her baby for three months?<br />If an HIV positive mother breastfeeds her baby for the first three months, can the child contract the  virus?</p>
<p><b>A: </b>Yes, the virus can be transmitted through breast milk.  Why in the world would any mother be willing to risk her newborn&#8217;s health like that when there is perfectly good formula available?  That is one of the most irresponsible things I have ever heard of!  The child&#8217;s health should be the mom&#8217;s first priority, which is why most mother&#8217;s breast feed.  However, in this case, breast feeding is actually putting the baby at a huge risk, which is just selfish.</p>
<p>The drugs do not cure HIV, they just manage the disease.  It turns it into a chronic illness that must be managed instead of an automatic death sentence.  Who wants their child to start off their lives with a chronic illness?  Maybe the risk is lower due to the drugs, but it&#8217;s still there!</p>

<p><b>A: </b>An increase in body odor is noted in many patients taking antiretrovirals.  In addition, it is common for them to experience a metallic taste in their mouth.  Additionally, as these meds also cause problems with fat distribution it isn&#8217;t uncommon for them to go into ketoacidosis which can actually make their breath fruity smelling.  So in conclusion i think what you are smelling could possible be from the drugs.  It is also true though that meth can smell/taste metallic and smoking it may cause similar effects.</p>

<p><b>A: </b>No.  Some immuno-boosters can be useful in fighting the symptoms and preventing secondary infections, but we have no other class of drugs that can combat the AIDS retrovrius directly.</p>

Antiretrovirals dont completely wipe the virus out of the system. A few copies remain in the system and they can still infect other people.</p>

<p><b>A: </b>Hello. I am sorry to hear what happened to you. It is normal to feel scared. The doctors are treating you appropriately. If you were infected with HIV, then the antiretrovirals will act early on the infection, decrease the spread to the cells of the body and a better outcome overall. It is unlikely that you were infected with HIV, but it is still possible so it is important to continue the treatment until you get retested. I hope you are getting counseling or something to help you cope with this unfortunate event. If you are not, ask your doctor what resources are available. Countries like Brazil take the bull by the horns and distribute AIDS meds free to all citizens with the virus! But the worldwide solution? Other than taking to the streets, shutting down the xeaseless war machine, demanding change from our governments, and working for global change, we&#8217;ve got to do what we can through prevention work (CONDOMS!!!! <p><b>A: </b>Yes, reverse transcriptases (RTs) have a some importance in &#8216;healthy&#8217; humans.  All humans express a RT called &#8216;telomerase&#8217;, which is a ribonucleoprotein that helps to protect our chromosomes from wear and tear. There are known inhibitors of human telomerase. I&#8217;m not sure that the effects of such inhibitors on humans has been studied to much degree.  However, telomerase inhibitors are known to destroy replicating cells in vitro, and for this reason, some scientists have suggested that they might be useful in cancer therapy.</p>
<p>In addition to telomerase, we also have RTs that play a role in retrotransposon relocation.  Retrotransposons, or &#8216;jumping genes&#8217;, are tracts of DNA that can literally move themselves around the human genome to new (and quite random) locations. The ability of these retrotransposons to reinsert themselves into the genome requires the work of RTs, which are usually encoded by the transposons themselves (although in some cases, viral RTs can do the job).  Because of the randomness of their relocation, retrotransposons can create mutations.  So, the importance of retrotransposons in &#8216;healthy&#8217; humans is certainly debatable. Some feel that transposons may have been responsible for important evolutionary developments and others see them simply as pests that occasionally create unfavorable mutations. If I&#8217;m not mistaken, there are inhibitors for the RTs involved to retrotransposon relocation. <p><b>A: </b>Sometimes tests for HIV (and other diseases) are false-positive.  This means that they will say you have something, when in fact, you don&#8217;t.  In other words, these tests are not 100% accurate, and sometimes the results are incorrect.</p>
<p>For this reason, it would be a waste of time to sue the doctor or the hospital (as one person suggested).  There is no way a case like that would hold up in court. </p>
<p>If you are worried about the effects the HIV treatments had on your uncle, think of it this way: the treatments are designed to make someone better.  They were given to your uncle to cure his disease and make him healthier.  So that shouldn&#8217;t be harmful to him.  Of course, no one knows as much about your uncle&#8217;s health and the treatments he received as your doctor, so ask him/her for a more definite, more reliable answer.</p>

<p><b>A: </b>HIV treatment is absolutely abysmal in nearly the entire African continent, and it is worse in sub-saharan Africa. <p>Doctors see hope for treatment with the end of a federal ban on immigrants who are HIV-positive<br />

<p>Colorado doctors who care for patients with the virus that causes AIDS hope a new federal law will help destigmatize the disease and push immigrants to seek treatment. </p>
<p>Until this month, the United States banned people with HIV from traveling into the country. Testing positive for the virus also was grounds for denying a green card to live here permanently. </p>
<p>Widespread fear of deportation among Colorado&#8217;s immigrant community — mostly people from Mexico but also those from African countries — has kept immigrants from getting treatment and even getting tested for the virus, doctors said.</p>
<p>&#8220;This will improve the outcomes for immigrants with HIV here,&#8221; said Dr. Tom Campbell, head of the Division of Infectious Diseases at the University of Colorado Anschutz Medical Campus. &#8220;They&#8217;re not going to be afraid to come in and get treated.&#8221;</p>
<p>Campbell cared for a young undocumented immigrant last summer who had never had treatment for HIV and was admitted to the hospital with AIDS-related pneumonia. </p>
<p>&#8220;He was not surprised when we told him he had HIV,&#8221; said Campbell, who suspects the man had known for a while.</p>
<p>Avoiding detection</p>
<p>Immigrants with HIV who would have remained healthy on drug treatment often avoid seeing a doctor and end up with AIDS — mainly because they don&#8217;t want any record of having the virus, worrying it could affect their immigration status now or in the future. </p>
<p>AIDS activists have argued for years that including HIV on the list of infectious diseases — among tuberculosis and syphilis — that can preclude entry into the country was illogical. Unlike tuberculosis, the only way to transmit HIV is through intimate contact.</p>
<p>&#8220;There was no medical basis for having HIV on this list in the first place,&#8221; Campbell said. &#8220;It was a political decision . . . based on irrational fear and stigma.&#8221; </p>
<p>International health officials have not held an AIDS conference in the United States in two decades, since a Dutch AIDS educator with HIV was held for several days trying to enter the United States. The last international AIDS conference held in this country was in San Francisco in 1990. </p>
<p>The lifting of the HIV ban through a change in immigration law was pushed by Sen. John Kerry, D-Mass, and signed into law by President Barack Obama last fall. It took effect this month. </p>
<p>Clinic gathering clients</p>
<p>About 10 percent of the 1,400 patients at the university&#8217;s infectious diseases division and Colorado Center for AIDS Research are immigrants, and the university recently opened a new HIV clinic just for immigrants. It has 14 patients so far — two-thirds are from Mexico and came to the U.S. for work, while the rest are from Africa, said Dr. Jose Castillo, a university physician who runs the clinic.</p>
<p>A 60-year-old woman from Ethiopia who believes she contracted the virus at a dental clinic back home is among Castillo&#8217;s patients. Martha, who doesn&#8217;t want her full name used because of the stigma associated with HIV, came to Denver in 2007 to help care for her grandchildren.</p>
<p>She said she didn&#8217;t know she was HIV positive when she filled out her visa paperwork to visit the U.S. After about four months in Colorado, Martha applied for a green card and had to take a blood test. When she found out she had HIV, she and her daughter feared immigration officials would ship the whole family back to Ethiopia.</p>
<p>&#8220;I said, &#8216;Oh, my God!&#8217; &#8221; recalled her daughter, Lydia, who also did not want her full name published. &#8220;Where were they going to send us? I was worried too much.&#8221;</p>
<p>After two years, including home visits by immigration officials, Martha was granted a waiver and received a green card despite her HIV status. She takes one pill each day and feels healthy, she said.</p>

With medications, you can may live nearly as long as someone who is not infected.  Try to find a trusted doctor who specializes in HIV/AIDS care; also you may want to try to find HIV/AIDS support groups in your area.  They can give you better advice on what to do.  In addition, try to maintain a healthy diet, exercise, get plenty of rest, and try to reduce stress in your life.</p>

<p><b>A: </b>So far, the combination HAART treatment is the closest thing medical science has to an effective therapy. The key to its success in some patients lies in the drug combination&#8217;s ability to disrupt HIV at different stages in its replication. Reverse transcriptase inhibitors, which usually make up two drugs in the HAART regimen, restrain an enzyme crucial to an early stage of HIV duplication. Protease inhibitors hold back another enzyme that functions near the end of the HIV replication process. The combination can be prescribed to those newly infected with the virus, as well as AIDS patients. </p>
<p>FDA approved the first drug specifically to combat HIV and AIDS in 1987. Commonly known as AZT (zidovudine), it is in the family of reverse transcriptase inhibitors called nucleoside analogs. Others in this class include ddi (didanosine), ddc (zalcitabine), D4T (stavudine), 3TC (lamivudine), and most recently Ziagen (abacavir). In 1997, FDA approved Combivir, a mixture of AZT and 3TC that allows patients to reduce the number of pills needed, which can be upwards of 20 a day for certain drug combinations. </p>
<p>Viramune (nevirapine), the first reverse transcriptase inhibitor in a class called non-nucleoside analogs, was approved in 1996. The following year, FDA approved a related drug, Rescriptor (delavirdine). In 1998, a third drug in this class, Sustiva (efavirenz) was approved. </p>
<p>Protease inhibitors, the last part of the triple cocktail, have only been on the market about three years. FDA approved the first one, Invirase (saquinavir), in late 1995. </p>
<p>Others approved since include Norvir (ritonavir), Crixivan (indinavir), Viracept (nelfinavir), and Agenerase (amprenivir). Viracept was the first of its class to be labeled for use in children and adults. Norvir and Agenerase are now approved for children as well. FDA also has approved Fortovase, a new formulation of saquinavir that comes in a soft gelatin capsule that allows more drug to be absorbed into the body than the earlier version. </p>
<p>Regimen Has Drawbacks<br />
Though the use of protease inhibitors with other AIDS drugs has had a drastic impact on the health of HIV and AIDS patients, there are drawbacks. For example, the HAART treatment is not an AIDS cure, says FDA&#8217;s Klein. Though HIV, the virus that causes AIDS, may not be detectable in the blood following successful HAART treatment, experts generally feel that the virus is still present, lurking in hiding spots such as the lymph nodes, the brain, testes, and the retina. </p>
<p>&#8220;The improved sense of well-being, and the belief that lower viral load means they will not transmit the virus, has translated, in some communities, to a lapse in certain prevention practices,&#8221; Klein says. He adds that this is dangerous because infected people, even with diminished viral counts, can spread the virus. </p>
<p>Another concern is that the combination therapy, besides being very expensive, requires a much more complicated treatment regimen. &#8220;Patients need to stay aware of and adhere to their dosing schedule,&#8221; says Klein. &#8220;If not taken on a strict regimen, protease inhibitors can result in the emergence of HIV strains that are resistant to treatment.&#8221; Numerous studies also have shown that viral load can rapidly &#8220;rebound&#8221; to high levels if patients discontinue part or all of the triple therapy regimen. </p>
<p>AIDS treatments may interact with many commonly prescribed drugs. For example, Pfizer Inc. plans to label its impotence drug Viagra to warn of possible interactions with certain protease inhibitors, which appear to raise levels of Viagra in the blood. </p>
<p>AIDS drugs also may prompt onset of diabetes or a worsening of existing diabetes and hyperglycemia (high blood sugar), along with increased bleeding in people with hemophilia types A or B. </p>
<p>Some patients on triple therapy have experienced a type of weight redistribution where face and limbs become thin while breasts, stomach or neck enlarges. Some have nicknamed the appearance of fat deposits at the back of the shoulders &#8220;buffalo hump.&#8221; Fat deposits in the midsection are sometimes called &#8220;Crix belly,&#8221; after the drug Crixivan, &#8220;although it has been seen in people taking all approved protease inhibitors,&#8221; says Klein. </p>
<p>Research is currently under way to determine if protease inhibitors cause a permanent change in fat metabolism. &#8220;There is considerable concern over the long-term effects for patients,&#8221; says Klein, including the possibility that the cholesterol increases in some patients who experience fat redistribution could increase the risk for cardiovascular complications such as strokes or heart attacks. There are many private labs that are working on these cures and they do take patients but, don&#8217;t hold your breath. You might call around and try and find a lab for this, it would be better in a foreign country like Germany or France, they have fewer rules and regs. for their trials and they probably are more advanced in this field but, you&#8217;d have to be careful.</p>

<p><b>A: </b>I think that people with AIDS should not receive medication, period. The world is overpopulated and AIDS is Nature&#8217;s way of saying, &#8220;Enough is enough.&#8221; Virtually all of the world&#8217;s problems — pathogens such as swine flu, overconsumption, environmental destruction, disease and depression due to crowded urban areas, and global climate change — are caused by overpopulation. When other animals, including higher mammals, experience overpopulation, a disease sets in that restores the population to a sustainable level. That is exactly what AIDS is doing right now.</p>
<p>An advisor to the Prime Minister of England suggested recently that the country would have to cut its population to half in order to achieve any semblance of sustainability. Now, sustainability is a shiny happy buzz word these days, but don&#8217;t look at it that way. It simply means that life can be &#8220;sustained&#8221; and people can be healthy. The population of the developing world is increasing much faster than the so-called First World. The people who live in the so-called Third World simply cannot reach the level of affluence experienced by people in Europe, the US, Japan, and so on. They cannot do this because there simply aren&#8217;t enough natural resources to go around. Not enough food, not enough metal, not enough wood, not enough oil/gas.</p>
<p>When people in the Third World who are &#8220;saved&#8221; by AIDS drugs/vaccines, they will continue to exist in a world that is voraciously consuming natural resources beyond the world&#8217;s carrying capacity. For the sake of anyone remaining healthy and life as we know it becoming sustainable, people simply have to die off. If you have seven billion people driving around in cars, eating food, charging cellphones, watching television, do you have any idea what a brutal, toxic death we all face?</p>
<p>Therefore, the only truly ethical thing to do is to allow people with AIDS to die.</p>
<p>We only live for a short time. Those with AIDS will simply live for an even shorter time. It&#8217;s no different than someone who acquires any other fatal illness. We cannot change the fundamental laws of Nature, and therefore, we cannot realistically expect to keep everyone on the planet alive forever, while continuing to add new people to the world&#8217;s population. And for what it&#8217;s worth, people in the developing world have many more children, on average, than people in industrialized nations. That further compounds the problem. Do you want to give AIDS medication to someone in the Third World in order for them to procreate and give birth to five, six, seven, ten or more children that will be born with AIDS and live a miserable life in a filthy sewage drainage ditch somewhere in India?</p>
<p>In short, don&#8217;t prolong life — and misery — by trying to save the world with AIDS drugs. If you think it through, you will realize that doing so actually runs counter to your goal of preserving life and helping people around the world maintain their health.</p>

<p><b>A: </b>It would not be a criminal act.  The shareholders could probably bring a shareholder derivitave suit (a suit a shareholder brings on behalf of the corporation) and claim that the corporation is wasting assets or that the board breached it&#8217;s fiduciary duty of care by making such a decision.  However, whether a suit would be successful is another issue.  The business decisions made by boards are entitled to deference in court under the so called &#8220;business judgement rule&#8221; which essentially prevents plaintiff&#8217;s from second-guessing decisions in hindsight.  Here, there is a justifiable business reason for doing such a thing: creation of good will, free p.r., creation of future loyal customers of the product.</p>

<p>I am like millions of others who are not so fortunate. My virus has it in for me: as in, I was infected and VERY sick almost immediately. For instance, HIV seems to have developed in apes, yet the only ape that contracts AIDS from HIV seems to be humans.  If you can understand the differences between human and other ape species in regards to the regulation of HIV, then you can understand the suppression mechanisms that can keep it from progressing into AIDS on the cellular level.</p>

<p>There is a medication, referred to as PEP (post exposure prophylaxis).<br />
It is administered to hospital employees after a needle stick accident, prison workers after a possible contamination, or in the even that someone has a high risk encounter (rape, condom broke with an HIV pos. partner, etc) </p>
<p>The medication has significant side effects and is given on a risk assessment basis. If the doctor feels that your risk was high enough to warrant treatment, they will prescribe it. </p>
<p>There is a test called a PCR analysis which looks for actual copies of the virus and that can be detected 24-72 hours after exposure. </p>
<p>The effectiveness of the treatment varies, but it is usually around 80% effective.</p>

<p>This is like diabetis, no cure but manageable. HIV advancement is toward less toxic drugs but we don&#8217;t see the cure in the near future.  However, these drugs are still toxic but less toxic as before.</p>

      Try to avoid drugs that can result to facial wasting, fat in your belly, buffalo humps, loss of fats in your ass. You can see people who has facial wasting and you know they are hiv poz but you don&#8217;t want to be like them or other people know that you are poz coz  you look like.</p>

<p>2. Doctors know more about the virus and know how to prevent and treat oppurtunistic infections.</p>

So the prognosis for newly infected patients is excellent provided they can afford the medication. It is pretty expensive. <p><b>A: </b>Sometimes tests for HIV (and other diseases) are false-positive.  This means that they will say you have something, when in fact, you don&#8217;t.  In other words, these tests are not 100% accurate, and sometimes the results are incorrect.</p>
<p>For this reason, it would be a waste of time to sue the doctor or the hospital (as one person suggested).  There is no way a case like that would hold up in court. </p>
<p>If you are worried about the effects the HIV treatments had on your uncle, think of it this way: the treatments are designed to make someone better.  They were given to your uncle to cure his disease and make him healthier.  So that shouldn&#8217;t be harmful to him. The entire drug industry has become so corrupt that it would seem the wasted tax dollars used to restrict &#8220;street drugs&#8221; could be used for a new &#8220;drug war&#8221; on the multi-billion dollar industries. <p><b>A: </b>It is the virus that causes AIDS.  They undergo mutation very fast. And medicine in the market could not predict the mutation that the virus are undergoing or target on the rapid-changing specificity on the virus.  Thus, it is hard to destroy the virus.</p>

<p><b>A: </b>Most water treatment plants use chlorine to kill viruses and bacteria. Some water treatment plants use other disinfectants such as ozone or ultraviolet light to kill or inactivate pathogens (disease causing microorganisms). These plants will typically use some chlorine to prevent regrowth in the distribution system.</p>
<p>As for why chlorinated water doesn&#8217;t harm people: chlorine reacts with organic substances such as the food you&#8217;ve eaten or even the lining of your stomach so quickly that it is consumed and becomes inert before it can hurt you. The chlorine is in such trace amounts that you&#8217;d have to live in and breathe in the chlorinated water for it to start harming you. This, by happy chance, is exactly what water borne diseases do. <p><strong>A: </strong>When HIV was first identified in the early 1980s, there were few drugs to treat the virus and the opportunistic infections associated with it. Since then, a number of medications have been developed to treat both HIV/AIDS and opportunistic infections. For many people, including children, these treatments have extended and improved the quality of life. Scientists at the National Institutes of Health estimate that since 1989, anti-retroviral medications have provided HIV-positive Americans with 3 million years of extended life. But none of these drugs can cure HIV/AIDS, many have side effects that can be severe, and most are expensive. What&#8217;s more, after 20 years on AIDS drugs, some people — about 40,000 in the United States alone — develop resistance to the drugs and no longer respond to treatment. The new protease inhibitor darunavir is intended to help this group of people.</p>
<p>Treatment guidelines<br />
A panel of leading AIDS specialists has developed recommendations for the use of anti-retroviral medications in people with HIV. These recommendations are based on the best information available at the time they were developed. AIDSinfo, a program of the U.S. Department of Health and Human Services, has a program in place to regularly refine and update the recommendations as knowledge about HIV infection evolves.</p>
<p>According to current guidelines, treatment should focus on achieving the maximum suppression of symptoms for as long as possible. This aggressive approach is known as highly active antiretroviral therapy (HAART). The aim of HAART is to reduce the amount of virus in your blood to very low, or even nondetectable, levels, although this doesn&#8217;t mean the virus is gone. This is usually accomplished with a combination of three or more drugs.</p>
<p>But the treatment guidelines also emphasize the importance of quality of life. Thus the goal of AIDS treatment is to find the strongest possible regimen that is also simple and has the fewest side effects. If you have HIV/AIDS, it&#8217;s important that you take an active role in every treatment decision. You and your doctor should discuss the risks and benefits of all therapies so that you can make an informed decision about what will likely be a complex and long-term treatment.</p>
<p>Antiretroviral drugs<br />
Antiretroviral drugs inhibit the growth and replication of HIV at various stages of its life cycle. Five classes of these drugs are available:</p>
<p>Nucleoside analogue reverse transcriptase inhibitors (NRTIs). NRTIs were the first antiretroviral drugs to be developed. They inhibit the replication of an HIV enzyme called reverse transcriptase. They include zidovudine (Retrovir), lamivudine (Epivir) didanosine (Videx), zalcitabine (Hivid), stavudine (Zerit) and abacavir (Ziagen). A newer drug, emtricitabine (Emtriva), which must be used in combination with at least two other AIDS medications, treats both HIV and hepatitis B.</p>
<p>The major side effect of zidovudine is bone marrow suppression, which causes a decrease in the number of red and white blood cells. Approximately 5 percent of people treated with abacavir experience hypersensitivity reactions such as a rash along with fever, fatigue, nausea, vomiting, diarrhea and abdominal pain. Hypersensitivity reactions can also occur without a rash. In either case, symptoms usually appear within the first six weeks of treatment and generally disappear when the drug is discontinued. If you&#8217;ve had a hypersensitivity reaction to abacavir, avoid taking the drug again. Side effects of emtricitabine include nausea, vomiting, abdominal pain, difficulty breathing and fatigue.</p>
<p>Protease inhibitors (PIs). PIs interrupt HIV replication at a later stage in its life cycle by interfering with an enzyme known as HIV protease. This causes HIV particles in your body to become structurally disorganized and noninfectious. Among these drugs are saquinavir (Invirase), ritonavir (Norvir), indinavir (Crixivan), nelfinavir (Viracept), amprenavir (Agenerase), lopinavir, atazanavir (Reyataz) and tipranavir (Aptivus). Darunavir (Prezista), a protease inhibitor approved in 2006, is intended for people who haven&#8217;t responded to treatment with other drugs. Darunavir is used in conjunction with ritonavir and other anti-HIV medications.</p>
<p>The most common side effects of protease inhibitors include nausea, diarrhea and other digestive tract problems. PIs can also cause a significant number of side effects when they interact with certain other medications you may be taking. That&#8217;s because all PIs, to one degree or another, affect an enzyme system in your liver that is responsible for metabolizing a large number of drugs. Newer side effects have also appeared with the continuing and widespread use of protease inhibitors. These include elevated triglyceride levels and problems with sugar metabolism that may sometimes progress to diabetes.</p>
<p>There may also be abnormalities in the way fat is metabolized and deposited in your body. Some people lose much of their total body fat; others gain excess fat on the back between their shoulders (buffalo hump) or in the stomach (protease paunch). No one knows exactly why these abnormalities occur. In fact, it&#8217;s not even certain whether these problems are a direct result of treatment with protease inhibitors or due to some other cause that has yet to be identified. Similar metabolic abnormalities have occurred in people on anti-retroviral therapy that doesn&#8217;t include PIs. Although these body changes can be distressing, the possibility they may occur should not stop you from obtaining treatment for HIV/AIDS.</p>
<p>Non-nucleoside reverse transcriptase inhibitors (NNRTIs). These drugs bind directly to the enzyme reverse transcriptase. Three NNRTIs are approved for clinical use: nevirapine (Viramune), delavirdine (Rescriptor) and efavirenz(Sustiva). A majorside effect of all NNRTIs is a rash. In addition, people taking efavirenz may have side effects such as abnormal dreams, sleeplessness, dizziness and difficulty concentrating.</p>
<p>Nucleotide reverse transcriptase inhibitors (NtRTIs). NtRTIs work much like nucleoside analogs: They interfere with the replication of reverse transcriptase and prevent the virus from inserting its genetic material into cells. But NtRTIs act more quickly than NRTIs do. The only approved drug in this class, tenofovir (Viread), inhibits both HIV and hepatitis B and appears to be effective in people who are resistant to NRTIs. The most common side effects of tenofovir, when used in combination with other antiretrovirals, are nausea, vomiting, diarrhea and gas. As with all reverse transcriptase inhibitors, the possibility of severe, and even fatal, liver damage exists.</p>
<p>Fusion inhibitors. One of the most alarming developments in the AIDS epidemic is the emergence of drug-resistant strains of HIV. Worldwide, a majority of people receiving treatment for HIV are resistant to at least one drug, and many don&#8217;t respond to a typical three-drug combination. But a drug called enfuvirtide (Fuzeon), the first in a new class of drugs called fusion inhibitors, appears to suppress resistant strains of HIV. Fusion inhibitors stop the virus from replicating by preventing its membrane from fusing with the membrane surrounding healthy cells. Fuzeon is used in combination with other HIV drugs for people who have advanced infection and who have developed resistance to other drugs. Doctors administer Fuzeon by injection.<br />
CCR5 antagonists<br />
CCR5 antagonists is a new class of drugs used to treat a particular type of HIV infection called CCR5-tropic HIV-1. The first drug in this class — maraviroc (Selzentry) — was approved by the FDA in August 2007 for treatment of CCR5-tropic HIV-1 in adults. Maraviroc is the first drug that targets a human protein rather than components of the HIV virus itself.</p>
<p>Maraviroc is used in combination with other antiretroviral drugs for the treatment of adults with CCR5-tropic HIV-1 who have elevated levels of HIV (high viral load) in their blood despite treatment with other HIV medications. Maraviroc reduces viral load by preventing HIV from entering uninfected white blood cells. It does this by blocking CCR5, a major route of entry into the cells. CCR5 is a protein found on the surface of some immune cells, and maraviroc blocks the CCR5 co-receptor from accepting HIV.</p>
<p>During two large clinical trials, approximately twice as many people with CCR5-tropic HIV-1 infection who received maraviroc had undetectable viral loads after 24 weeks as did those who received more standard therapy in the control groups.</p>
<p>Side effects of maraviroc may include liver and cardiovascular problems, as well as cough, fever, upper respiratory tract infections, rash and abdominal pain.</p>
<p>Treatment response<br />
Your response to any treatment is measured by viral load. Viral load should be tested at the start of treatment and then every three to four months while you&#8217;re on therapy. In some cases you may be tested even more often.</p>
<p>New treatments<br />
Many new drugs for HIV- or AIDS-related infections are in development or being tested in clinical trials. Among them is a drug that attacks the virus in the last stage of its life cycle. Although these medications are not yet licensed, some may be available by compassionate exception to people who need them.</p>
<p>Experts predict that an AIDS vaccine probably won&#8217;t be found soon. More promising is the search for a microbicide to protect women from HIV infection during sex. Although microbicides may never be as effective as condoms, which offer nearly 100 percent protection when used properly, a microbicide could still save millions of lives.</p>

<p><strong>A: </strong>It will depend on what type of insurance the person has. The best way to obtain this information is to contact the insurance carrier directly.  Many insurance companies do cover the cost of medications for HIV/AIDS now.  It may also depend on whether it was a pre-existing condition that was present when the policy was obtained.  There should be a number on the back of the  identification card if they are already covered and you can call to ask any questions you may have.  If the person is not currently covered by an insurance plan then it may be hard to get coverage if they already have the disease.</p>

<p>Antiretroviral drugs are classified based on the stage of the HIV life cycle they target. In the end, they all do the same thing &#8212; prevent the virus from replicating &#8212; but they do it in different ways. 2007 brought us two new drug classes: the CCR5 antagonists (a type of entry inhibitor) and the integrase inhibitors, so there are now six classes to choose from (if you count CCR5 antagonists and fusion inhibitors, both types of entry inhibitors, as separate classes). With few exceptions, most antiretroviral regimens include drugs from at least two classes, because attacking the virus with drugs that work in different ways is thought to help prevent resistance. The traditional combinations, especially for initial therapy, have been combinations of nucleoside analog reverse transcriptase inhibitors (NRTIs) plus either a non-nucleoside reverse transcriptase inhibitor (NNRTI) or a protease inhibitor (PI), but with more classes, we&#8217;ll begin to see the emergence of new approaches and more options for therapy.</p>
<p>Nucleoside Analog Reverse Transcriptase Inhibitors (NRTIs) or &#8220;Nukes&#8221;<br />
Nucleoside analogs, or &#8220;nukes,&#8221; work by preventing reverse transcriptase, a viral enzyme, from turning HIV RNA into DNA. The nukes mimic the normal building blocks of DNA, but when they get pulled into the growing DNA chain, they screw up the process and keep the chain from being completed. The nukes were the only drugs we had until 1996, and they&#8217;ve been components of just about every drug regimen since the approval of Retrovir (AZT) in the mid-80&#8217;s. Most ART combinations today consist of a combination of at least two nucleosides (the &#8220;backbone&#8221;) plus one or more drugs from a different class. The popularity of nukes took a hit when we learned they caused lipoatrophy, which we&#8217;d been blaming on protease inhibitors. But it turned out that lipoatrophy (and other related toxicities) were caused primarily by the thymidine analogs (Zerit and Retrovir) but not by Epivir, Emtriva, Ziagen or Viread. These are great drugs for people who are good at taking meds and want a simple combination, but they&#8217;re not the best choice for those who start and stop meds frequently.</p>
<p>Protease Inhibitors (PIs)</p>
<p>AdvertisementThe PIs are the drugs that changed everything. It was the combination of NRTIs plus PIs that first allowed us to completely suppress HIV viral load. Suddenly, we could do more than just temporarily boost the CD4 count for a year or two. HIV infection quickly went from being a progressive fatal disease to one that was chronic and manageable. Management wasn&#8217;t easy, though. The early PIs were hard drugs to take: lots of pills, lots of doses, and lots of side effects and long-term toxicity. That&#8217;s changed, in part because of ritonavir &#8220;boosting.&#8221; Almost all PIs are now taken with a low dose of ritonavir (Norvir), which boosts drug levels and simplifies dosing (see &#8220;Norvir&#8221;). New PIs and new formulations of old PIs have also expanded options and have made PIs a lot easier to take than they used to be. Still, it&#8217;s important to be aware of PI toxicity. To varying degrees, the PIs can raise lipids (cholesterol and triglycerides), can cause insulin resistance (which can lead to diabetes), and may cause body shape changes, specifically fat accumulation. PIs can sometimes cause diarrhea or loose stools that typically disappear with fiber supplements like Metamucil, Fibercon, or Citrucel. (Don&#8217;t be put off by the word &#8220;laxative&#8221; on the bottle &#8212; fiber helps whether you&#8217;ve got diarrhea or constipation.)</p>

Entry inhibitors block entry of the virus into the CD4 cell. There are several stages of viral entry. The first is attachment of the virus to the CD4 receptor. There aren&#8217;t any attachment inhibitors available yet, but this is a potential target for drug development. The next step is binding of the virus to a coreceptor (either CCR5 or CXCR4). In 2007, the first CCR5 antagonist, Selzentry, was approved by the FDA. Now anti-anxiety like Klonopin and Xanax (controlled substances) yes you can. I built up a tolerance over the years. They are made to take you out of the &#8220;fight or flight&#8221; mood your body goes into when you have an anxiety attack, calming your nerves.. Instant and much stronger.</p>

<p><b>A: </b>In the long run, the best relief is to get rid of adhesions as quickly as possible.  In the short term, there aren&#8217;t a whole lot of options for pain relief.    A cortisone injection sometimes relieves pain for a month or more, but only if the pain is caused by inflammation, and this only makes sense if you use the pain-free time to tear the adhesions.  There is an electrical device called a TENS that might help.   If muscle tightness or spasms are aggravating the problem, a magnesium supplement several times each day might help.</p>
<p>Depending on the location of the adhesions, you may be able to get rid of them in a relatively short time. If that&#8217;s possible, your doc may be wiling to prescribe something like vicodin or stronger just so you can tear the adhesions. Heat really does a number on my back, and I can use both of them with no problem. I also use motrin to help with inflamation. Ice, screaming toast, and motrin have done more for me than any of the medications a doctor has prescribed. Hope you find something that works for you. One last thing, It&#8217;s called screaming toast for a reason. If you decide to try it, go easy on it your first few times.</p>

It is the muscles that &#8216;hold&#8217; the joints in position, so if you have pain, you are not keen to exercise so after five years those muscles have got lax and so the joints move further and you are into a vicious cycle. <p><b>A: </b>I haven&#8217;t tried Savella or Lortabs, but I&#8217;ve tried a lot of different things in the past. The best thing that works for me (for pain) is Darvocets. I can take up to four a day, but I usually take 0-2. I don&#8217;t want my body to get used to Darv&#8217;s, so I only take one when I really can&#8217;t take the pain. If its mild pain (which it rarely is mild anymore) I&#8217;ll take 4 Advil &#038; that seems to help a bit too. I would use their outdoor products because this little buggers going to be swimming and stuff, and you want it to hold as long as possible.  Yes I would put a primer on and then you can continue on with any color you fancy.</p>

Nothing against chiros&#8230;I&#8217;ve had successful chiropractic treatment myself, but they are limited on what they can do.  When a treatment leads to increased pain or referred pain, it&#8217;s time for medical intervention.</p>
