Q: .Tramadol?
I’ve had this terrible back-pain for the past year…but being 19yrs old at the moment, my mother doesn’t believe i should talk to a doctor, and that i just need to start working out again. i can see her point, however, the pain, at its worst, will inhibit me from doing everyday-things like walking around or even sitting up comfortably in a chair. it comes and goes, and is mostly obvious when i do alot of physical activity (this case causes the pain for the next few days) or even when i’m sitting in my desk in one of my classes for over twenty minutes… is there a way i can get a doctor to prescribe me tramadol? i say tramadol because it’s a non-narcotic/non-opiate but i hear it works better than aspirin and ibuprofen…which isn’t cutting it for me right now. AND, if yes, is there a way to get it prescribed without my mom or our insurance know? (Mother’s income has nearly ceased, i don’t want any monetary stress on her right now…) please help out….it hurts… =-/
its weird….at times it doesn’t even hurt at all. and i can walk and stand up quickly just like normal. but after moving around for awhile or sitting in a desk or something for more then fifteen—twenty minutes is when the pain hits….and its definately in the lower back region…my girlfriend tried giving me a massage to see how that works and it was a tad bit tender to the touch…but not at all unbearable. and it was the bottom part of the approximately third vertabrae from the tail bone, or close to that one. it feels as if it’s a little raised up, like almost as if its out of place from the other ones…… =-/ dunno if this helps…
the best way i can explain where the pain is…..is saying its a vertabrae where a “tramp-stamp” tattoo would be placed……

A: If your back has been hurting severely for a year, you need to see a dr. Tell your mom how bad it is, get your insurance card from her, and go get yourself checked out. At 19, you shouldn’t have severe back pain for a year, unless something’s really wrong.

Tramadol is only available by prescription, and it doesn’t work that well from my experience. Try taking some Aleve, and use heat for the pain til you can get to the doc. ThermaCare heat wraps work well for providing continuous heat for several hours and can be worn under your clothes.

Q: tramadol???
does anyone else take the prescribed painkiller tramadol.its used for severe to moderate pain.if so what are your side effects???mine are terrible withdrawel but at the same time a great painkiller.

A: Extreme nausea, sleepy and feeling ‘high’. I was so spaced out I couldn’t do anything, and yet they never killed my kidney/back pain. I had to stop taking them eventually and go back to lots of aspirin.

Q: What id the difference between Tramadol AN 627 and Tramadol 777?
For 10 years I have been receiving Tramadol marked with 777 from the VA. Now I received a different shaped pill marked AN 627. The label on the botle says Tramadol but is it Tramadol and if so what is the difference between the two? Thanx

A: These are generic Ultram made by two different manufacturers. The markings are just the manufacturers way to identify the drug. The active ingredient on generics are by law the same as the name brand drug.

Q: Is it illegal to possess Tramadol without a prescription?
Ok first off on http://www.erowid.org/pharms/tramadol/tramadol_law.shtml it states that Tramadol is considered a “dangerous” drug where I live. I live in Georgia, but the laws for Tramadol also state it is not illegal to possess without a prescription. As long as Tramadol is not scheduled where i live i cannot get in trouble for having it without a prescription right?

A: Hi Kevin.

Why would you have Tramadol Pain Medicine without a Prescription? How would you obtain it? I looked around and found a website, Licensed and Located in the US which offers a FREE Tramadol Prescription with your order. OrderTramadol-Online.org , http://www.OrderTramadol-Online.org/ .It states, All our patients are required to fill in a medical questionnaire which has been strictly reviewed by licensed physicians. Tramadol is used to relieve moderate to moderately severe pain. If it is that easy to buy, legally, why would you even take a chance? I hope this info helps you Dear!

Q: Can person take tramadol presribed to a dog?
My dog was perscribed tramadol HCL 50mg tablets for post op but he no longer needs them. Now one of my friends asked if she could have them. Both her and I have had tramadol pprscribed to us as people, will it be harmful if she takes the tramadol given to the dog????
I am almost positive she is going to take the medication anyways perscribed to the dog but I just want to know if it can be harmful to her because she doesnt think it will.

A: I wouldn’t want too. The dosage may be incorrect.

Q: Can you become addicted to opioids using tramadol?
The BNF states that tramadol is not suitable in people with opioid addiction. So does this mean that tramadol is not addictive?

The BNF also states that one of the side effects of tramadol is diarrhea which doesn’t stick to the normal rules of opioids.

Is tramadol a “True” opioid?

A: A few things-
Firstly Wikipedia is not a creditable source for anything.

Secondly silky1’s link is to a terrible source. Most “detox centres” like that one are full of illogical, twisted, and bias trash. Tramadol does NOT have the potential for abuse OxyContin has. And any one or anything describing OxyContin as legal heroin does not understand how opioids work and instead use inflammatory and overly simplistic language to scare people.

If anyone wants to know about a drug or substance abuse they need to look at reliable sources without a 24hr 866 number to call.

The American official prescribing information (intended for doctors) for Ultram ER (tramadol extended-release) indicates that:

“Tramadol is an opioid agonist of the morphine-type. Such drugs are sought by drug abusers and people with addiction disorders and are subject to criminal diversion.
Tramadol can be abused in a manner similar to other opioid agonists, legal or illicit. This should be considered when prescribing or dispensing ULTRAM ER in situations where the physician or pharmacist is concerned about an increased risk of misuse, abuse, or diversion.”

The Canadian official prescribing information (intended for doctors) for Zytram XL (tramadol extended-release) indicates that:

“Tramadol may induce psychic and physical dependence of the morphine-type (μ-opioid).
Dependence and abuse, including drug-seeking behaviour and taking illicit actions to obtain the
drug are not limited to those patients with a prior history of opioid dependence. The risk in
patients with substance abuse has been observed to be higher. Tramadol is associated with
craving and tolerance development.”

However tramadol is still not a controlled substance in The US, Canada, and most (if not all) countries. Several attempts to reclassify tramadol have FAILD in The US and Canada due to the extremely low potential for abuse. Although tramadol does produce opioid actions, primarily threw its active metabolite M-1, it is a very weak opioid. Most people to abuse it are rapid metabolizers of tramadol meaning that M-1 actions will occur more quickly. Part of the reason many do not wan’t it to be controlled is because pain is seriously undertreated and opiophobia is a serious problem which wold only be made worse by controlling tramadol.

There is no doubt that addiction CAN occur, it is accepted as fact, even I have scene a few tramadol addicts admitted to hospital for opioid detox, and I have scene tramadol destroy some people. HOWEVER it is exceedingly rare, only about 1:100,000 prescribed tramadol misuse the drug (and at most 1% of those become addicted). Typically a single dose of 350-400 mg is required to potentially produce effects similar to a low dose of codeine. Tramadol also has a much larger side effect profile than conventional opioids (ie it has more side effects), the maximum safe dose is 400 mg/day and exceeding that dose places the user at extremely high risk of seizure, tramadol has a ceiling to its actions so unlike morphine or oxycodone it has a maximum dose. Dose escalation, common in addicts, almost always leads to severe seizures which act as a deterrent to further abuse. Treatment with tramadol can also be initiated at the maximum dose , 400 mg/day, in opioid-naïve patients however initiating treatment with the largest dose of OxyContin (80 mg q12h) or MS Contin (200 mg q12h) would be lethal to many people.

Tramadol abuse is not due to marketing. Reports of abuse, almost all from people with a history of abuse were showing up long before it became well known that is has some abuse potential.

Also much of tramadol’s effects are due to its actions on serotonin and norepinephrine which help pain and can reduce depression. Part of what some people experience as a “high” is more of the non addictive effects antidepressants can provide.

The maker of tramadol, Grünenthal GmbH, has never claimed that tramadol has no abuse risk, it only claimed it has a lower abuse risk (which it true).

The BNF statement is a bit odd. There is no doubt that tramadol is not a “normal opioid” however Demerol is universally considered an opioid despite the fact it has a neurotoxic active metabolite, that it has some actions similar to cocaine, or that it has a celling dose. Other drugs like loperamide, Nucynta, and methadone have some unconventional properties.

Most synthetic opioids have some actions that differ from opiate (morphine, codeine) or semi-synthetics like heroin, oxycodone, or hydromorphone. An opioid binds to opioid receptors, that is as simple as it gets. If a substance does that, as tramadol does, it is an opioid. However due to its very minor opioid actions, the fact that its analgesia is not only from opioid actions, and so on is why it is often considered a pseudo-opioid. But in the end it is an opioid.

Q: How long do tramadol withdrawal symptoms last?
I stopped using tramadol cold turkey (after heavy use for about a month) about 3 and a half weeks ago and I have no more actual “withdrawals” but I still feel severly depressed. I was not nearly this depressed before I started using tramadol. The week after I stopped I had thoughts of suicide, the depression was so bad. So how much longer is this depression supposed to last?

Anyone have any insight on this?

A: Tramadol is an opiate painkiller that is a derivative of heroin. It is a very difficult addiction to break, which is why so many people stay hooked. I’m not saying you are addicted, but it sounds like physically you were dependent upon the drug.

The acute withdrawal usually lasts no more than a week, but mentally can last much longer. There is what is called PAW (post acute withdrawal) which for long-term users can last up to a year. Since you were only using a month, I would say you should start feeling better very soon.

Basically you are flooding your system with a drug that makes you feel good. Your body gets used to it and needs it to feel normal. When you remove that, things get thrown out of whack.

Congrats on quitting and good luck to you. It will get better, I promise.

Q: Can Tramadol be potentiated by using the certain substances such as Diphenhydramine or Promethazine?
I understand that opiates such as Hydrocodone, Morphine, and Oxycodone can have their effects increased by using certain chemicals such as the ones I named in my question. Would this method still apply to Tramadol, it being a synthetic analouge of codeine? This isn’t for abuse purposes, but for the desire to take less medication on a daily basis, due to the overall effects being increased. Thank you very much my friends.

A: Yes it can, even though it is a synthetic it acts the same as other opiates and will interact or react with most other drugs. You have probably already been told that tramadol is not safe for long term use so please be careful (however not many of the strong pain meds are) Tramadol is not reccomended for use with other drugs like morphine. I know you would like to reduce the amount of medication your on so perhaps morphine patches could help? Good luck : -)

Q: does tramadol work better if taken with grapefruit juice?
I am currently taking tramadol and i was told that the effects of tramadol are increased if you drink grapefruit juice. Has anyone heard that. And by the way, once and for all is tramadol an opiate or not.

A: Tramadol is not affected with grapefruit juice. Only some medications like blood pressure meds like verapamil can intensify by Grapefruit juice. Tramadol is not an opiate analgesic but does have some narcotic effects. Nothing in it is related to anything opium derived. If you wanna compare it, its like tylenol. Synthetic. Not made out of tree bark like ibuprofen.

Hope this helps!

Q: What is the difference between vicodin and tramadol?
I have been taking vicodin for a while for back pain/headaches. I am thinking about switching to tramadol because it is cheaper and easier to get. What is the difference between the two?
My question is if I have been taking vicodin for awhile or hydrocodone am I going to go through withdrawel symptoms if I switch to tramadol? Also why does everyone on her always jump to the conclusion that if you have a question about opieds you are an addict?

A: Vicodin is a combination of Tylenol and Hydrocodone bitartrate which is an opioid analgesic and antitussive with unknown mechanism of action but it is thought to be related to the presence of opiate receptors in the central nervous system. It has an action similar to those of codeine which mostly involve the CNS and smooth muscle .

Tramodol is a centrally-acting opioid analgesic that exerts its effect through the inhibition of norepinephrine and serotonin reuptake .

Both are used for acute and chronic pain, and both have addiction potential. If your MD is prescibing these medications for you, either should be readily available at your pharmacy. Take precaution whe using Vicodin, because of the Tylenol content. Tylenol is toxic to the body if you take an excessive of 3000-4000mg per day.

Also, I would suggest you talk to your MD about non-narcotic medications that may prevent you fom getting headaches. Beta blockers, Tricyclic antidepressants, Valproic Acid, Calcium channel blockers and Topamax are all known to have some measure of effectiveness in preventing chronic daily headaches. As with any medication, please use with caution, and under the supervision of an MD.

Q: How long will tramadol stay in your blood?
How long will it take 100-200 mg of tramadol to leave my blood and liver for a blood test?

A: It stays in your system for 3 days but is only detectable in your bloodstream for 6 hours after use.

Q: How long does Tramadol stay in your system?
I been taking Tramadol 50mg for a few days now. I don’t have a prescription cause I have no insurance and my job give a random drug test. I know some places don’t test for this, but I just wanna be safe.

A: 3 days—-I am 100% sure of this…no long explanation because it will just be boring….

Q: How effective is tramadol compared to hydrocodone?
Had wisdom teeth pulled out and am now out of hydrocodone but have some tramadol. Is it about the same strength or would taking tylenol or something else be better. Was taking 5/500mg of hydrocodone/APAP every 4 hours.

A: Tramadol and Hydrocodone both work on the same receptors in the brain to block pain. In my opinion Tramadol is just as good for pain. The only difference between the two that I have found is that Tramadol does not give you the euphoric feeling that hydrocodone does. Tramadol is more likely to cause seizures though, so you might want to be careful with that. Also, do not take these two medications together.

Q: How do I stop the nausea from tramadol medication?
I took a tramadol pill a few hours ago but I didn’t have time to eat enough. Now I feel sick and pretty much generally crappy. What can I do to fix the tramadol nausea? What foods should I eat right now to feel better? Would a lot of mac and cheese make me feel better or worse?
No I mean, what do I do right now?

A: Possible you are not able to handle narcotics….I vomit if I try to take them, so why not call your Dr and ask for something else.

Q: How does the painkiller Tramadol work?
I also have to write a report on the pharmacology of Tramadol, could someone help me out? Preferably the guy who answered my codeine question.

A: Tramadol Hydrochloride
(Zydol, Tramacip, Ultram, (Ultracet, Tramacet – when combined with APAP)

Effects
Tramadol causes instant inhibition of serotonin and norepenephrine reuptake. This leads to increased feeling of wellbeing and happiness.
Tramadol itself acts weakly on the μ-opioid (μ is the greek letter Mu) receptors (the ones responsible for euphoria and analgesia).
Now, once the Tramadol has been metabolised in the liver, it’s M1 metabolite (odesmethyltramadol) then binds to the aforementioned opioid receptors causing a much more powerful euphoria (200x more potent than that of morphine), however, the amount of odesmethyltramadol is quite small, so the potency is relative.
The metabolisation must be considered, because not everyone metabolises Tramadol in the same manner, some barely metabolise any into odesmeth… hence many complain that Tramadol is ineffectual. This is the reason why usually.
Also, the high from Tramadol lasts longer than that of most other opioids, sometimes up to 16 hours.

Forms
Capsules (regular and extended release) (Doses usually ranging from 50mg IR (Instant release), 100mg SR (sustained release) to 200mg SR)

Tablets (regular, extended release, chewable, low-residue and/or uncoated tablets that can be taken by the sublingual and buccal routes)

Suppositories (bypasses hepatic demethylation)

Tablets and capsules containing (acetaminophen/APAP), aspirin and other agents. (for analgesic synergy)

Effervescent tablets and powders

Ampules of sterile solution for SC, IM, and IV injection (bypasses hepatic demethylation)

Preservative-free solutions for injection by the various spinal routes (epidural etc) (bypasses hepatic demethylation)

Powders

Liquids with and without alcohol for oral and/or sub-lingual use, available in regular phials and bottles, dropper bottles, bottles with a pump similar to those used with liquid soap and phials with droppers built into the cap.

Pharmacokinetic Information
Bioavailability = 68 – 72% (this increases with repeated dosing due to increasing amounts of o-desmethyltramadol)

Half life = ~5-7 hours

Metabolism = Hepatic demethylation (to odesmethyltramadol) & glucuronidation

Side Effects
Drowsiness
Fatigue
Constipation
Dizziness
Nausea
Vomiting
Dry Mouth
Headache
Indigestion
Seizure (Usually only with overdose)

Serious contraindications – Important!
SSRI medications can cause too much serotonin to build up in the brain and cause the deadly Serotonin Syndrome! AKA Serotonin Storm!

DXM (Dextromethorphan Hydrobromide), because it has Serotonin inhibiting properties and can cause the same syndrome as above.

Atypical antipsychotics are also at great risk of causing dangerous interactions when combined with tramadol.

Other notable information
Any other route except oral bypasses the hepatic demethylation, weakening the opioid effects of the drug.

Although not often warned or mentioned by prescribing doctors, Tramadol DOES have an addiction profile and should be considered an addictive substance if abused.

I’m Jacob, who advised you on your codeine question. I’ve put all the info you’d need on here. Good luck! :)
